How to Request an Appointment
Go to USave Portal Web Page
Click USave Health Portal Button
Click "I'M A NEW CLIENT" or "I’M AN EXISTING CLIENT"
Select Service
Select (location) Video Office
Select date on Calendar
Choose time
Select Reason for care
Click Next
Answer Questions
Upload form of ID (Driver’s license)
Click Next
Choose Payment method (Out of Pocket)
Click Next
Click Preferred time
Click Next
Enter Contact Information
Click Continue & Review
View & Confirm
Click Request Appointment
Then Check your email inbox for Appointment Confirmation
EXISTING CLIENT
Go to USave Portal Web Page
Click USave Health Portal Button
Click "I’M AN EXISTING CLIENT"
Enter your email address
Click Send Link
Check your email (inbox)
Click SIGN IN
Click STAY ON WEB (or continue to go to download Simple Practice app)
Sign in
Agree to terms
Click SUBMIT
Click STAY ON WEB
Click on Consent for Telehealth Consultation form
Read, scroll down and sign
Click SAVE CHANGES
Click CONTINUE
Sign Credit Card Authorization
Click CONTINUE
Sign Consent to Record Audio
Click Continue
Sign Consent for Use of AI Services
Submit
Upload any additional documents if needed
Click Menu
Sign out
Then Check your inbox [Video Appointment confirmed]
[Requested appointment will be pending confirmation & approval. You will be notified via email when an appointment has been confirmed. You can add the appointment to your calendar. You must call to cancel your appointment]

Health History Intake
New Patient Intake & Registration

upload any necessary files
Click Pay Now Button
Make a payment in Full
Pay with Card
Pay with link
Enter payment information
Click Pay [You will receive an automated message]
Go to your email
Click the Video Appointment link
SIGN INTO the Client Portal
Start Virtual Visit
